Fentanyl is generally the safest opioid for patients with renal and liver impairment. Other opioids, such as morphine, tramadol, oxycodone, etc, can still be used safely but require careful adjustment. Ensure longer dosing intervals, closely monitor for side effects (such as excessive sedation or toxicity), and avoid prolonged-release formulations.
